The Your Medicines @Home Service works to develop seamless pathways of care and support for patients who have problems in managing their medicines. The team supports patients at home and:
The post holder will work with the Band 5 Pharmacy Technicians, the Band 6 Pharmacy Technician and the Band 8a Clinical Lead Pharmacist to continue to develop the Your Medicines@Home service across the Merthyr Tydfil and Rhondda Cynon Taf localities to extend access to further health and social care colleagues, and will support the Clinical Lead Pharmacist in leading the team.
This role will be key to be responsive to patient’s needs within their localities by working in collaboration with the GP practices and professionals with the cluster teams in getting involved at the very earliest point support patients in maintaining their independence within their own home. The team is ambitious in looking to develop innovative and creative ways to support the individual and those important to them. It works to recognise the changing needs within communities and works collaboratively to meet them.

This service has won a number of NHS awards, in collaboration with the Stay Well@Home Service that is the umbrella service for supporting patients in maintaining independence following a hospital discharge.
