An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join our dynamic and compassionate team at Singleton Hospital, which hosts the South West Wales Cancer Centre. Our leading Cancer Centre specialises in providing comprehensive, patient-centred inpatient and outpatient Systemic Anti-Cancer Therapy (SACT), radiotherapy services, and autologous stem cell transplant as part of the South Wales Blood and Marrow Transplant Programme.

The successful candidate with a current level of experience that supports appointment to a clinical pharmacist role but requires further training to support an advanced practice role will be appointed to the Band 7 for a minimum 12-month period.
The candidate will follow a structured training programme with support from senior mentors and will focus on building and developing key skills, aptitudes and competency through experience, feedback and assessment.
On successful attainment of the competencies to the required level, the candidate will be appointed to an Advanced Pharmacist Band 8a and will work to the Job Description commensurate to that post.
Our Pharmacy Cancer Team are well integrated into the multidisciplinary team and operate a wide range of non‑medical prescriber clinics and organisation of homecare services. As a Specialist Cancer Services Pharmacist, this role will be pivotal to the provision and development of an advanced pharmaceutical care service for haematology and oncology patients.
This is an ideal role for a pharmacist wanting experience of providing care to cancer patients and who is keen to step up in a supportive, innovative working environment. Specific cancer experience is desirable but not essential. You will work with our cancer patients on the wards and in various outpatient settings.
Your experience will be gained working in a patient focussed environment using your knowledge and skills to improve patient care and promote optimal outcomes from medicines, including experience of undertaking research and/or audit. You will work as part of the pharmacy team to provide dispensary support to our outpatients and support the education and training of junior staff.
In addition, the role will require the ongoing completion of core directorate activities such as reporting and analysis of medicines expenditure, clinical governance and risk management.
Provide expert advice and guidance on the safe and effective use of medicines and produce SACT related policies and shared care protocols.
Undertake clinical reviews of cancer patients receiving SACT in an outpatient setting.
Represent pharmacy and provide clinical pharmaceutical expertise at multidisciplinary team meetings.
Provide education and clinical leadership to junior pharmacists and trainees, and other health professionals/undergraduates.
Investigate medication related incidents or complaints within haematology / oncology services and explore ways of minimising the risk of future re occurrence.
Represent pharmacy at directorate/service meetings within Cancer Services.
