Job summary

An exciting opportunity has arisen for an experienced nurse to become the Clinical Team Lead of Halton Wellbeing, a community health service based at The Bridges in Widnes. The successful post holder will have both clinical and operational responsibilities and must be able to demonstrate strong leadership and excellent clinical skills to support and lead the team to deliver high quality clinical care.

We are looking for someone who is dynamic and motivated, and committed to providing visible daily leadership and clinical expertise to the team to ensure that the service provided meets local and national targets. The post holder will have highly developed communication skills enabling them to build effective working relationships with stakeholders and promote successful partnership working.

Main duties of the job

The post holder is responsible for the day-to-day running of the Wellbeing Service providing line management and clinical leadership to the team. Responsibilities include:

Coordinating and leading the team to provide health assessments for patients on the SMI register and ensuring that monthly targets are met.

Performance monitoring and management including engaging with the commissioner and service evaluators.

Working with the team to ensure that standards of patient care are high and to maintain safety and quality of the patient experience.

Using your specialist knowledge and experience to provide a high-quality nursing service for patients with SMI to assess and advise on physical health care including diet, weight gain, exercise, lifestyle, sexual health, and medication monitoring.

Leading and undertaking clinical audits.

Liaising with GP practices in Halton to ensure targets are achieved.
