We are looking for an experienced nurse to take up the Secondment post of Ward Manager to support the development of the following priorities & to support & drive a new model forward to deliver evidence-based practice to ensure we deliver the best patient outcomes.
-To provide outstanding step down & step-up rehabilitation
-To provide step-up care from the community, preventing admission to secondary care
-To provide palliative ward-based care for those at the end of their life
-To support the delivery of place-based care for people living in Mid & North Hampshire.
Alton Community Hospital works closely with GPs & local partners to ensure the population of Mid & North Hampshire have access to first class rehabilitation & step-up care. You will work closely with the AHP manager to both support step up care, preventing unnecessary acute admissions, & drive an outstanding rehabilitation model. You will lead the wider MDT, incorporating both internal & external partners, to provide the population of Mid & North Hampshire with seamless health care services with a home first approach.

To maintain responsibility for the service to ensure safety over a 24 hour period, with a focus on leadership and operational responsibility whilst offering clinical support alongside the Consultant Practitioner, Clinical Services Matron, Trainee Advanced Clinical Practitioners.
