The Urgent Community Response service is a key part of the integrated urgent care system for Lincolnshire by providing virtual and face to face assessment of patients in their own home experiencing an acute need that requires a 2 hour response.
An exciting opportunity has arisen to recruit to the existing multi-disciplinary team that is based across the county.
The successful post holder will be responsible for the assessment of health and clinical needs, in line with the UCR standards, by utilising their professional clinical judgement.
There is opportunity for the role to based at either of our bases (Lincoln, Boston, Spalding, Louth and Sleaford). The service runs 8-8, 365 days a year. There is a requirement to work a variety of shifts to cover the service opening hours and this includes weekend work.
The successful candidate will be expected:
To work as an accountable and autonomous practitioner within the Urgent Community Response team delivering patient centred care, with particular focus on assessment of need, coordination of services, and planning that will enable individuals to avoid admission to the acute setting, ambulance call out/conveyance.
To respond to people across Lincolnshire who require timely, intervention and assessment of need and be responsible for liaison with other services to support patients to stay in their own home.
Physiotherapists, Occupational therapists, Nurses and paramedics can apply.
Main duties of the job
- Independently assess patients referred into the service using a specialist assessment to determine clinical diagnosis and treatment indicated and maintain records as an autonomous registered therapist within a variety of clinical settings.
- Use a wide range of skills and strategies to communicate with people about difficult matters or situations
- To manage a clinical caseload of patients with complex and specialised needs to a high professional standard and seek guidance from senior colleagues when necessary
- The job role requires frequent moderate physical effort e.g. moving and handling patients, mobile equipment, using hoists and other mechanical aids.
- Car driving daily, varying distances in urban and rural communities, including during adverse weather conditions

- Use skills of critical analysis and evaluation to make justifiable and timely clinical judgements utilising appropriate and sometimes limited information from a wide range of sources to assess, diagnose, plan, implement or direct complex care/interventions and evaluate effectiveness
- Identify and support the achievement of learning needs of individuals/team in response to service need and personal development planning
- Actively promote a supportive culture, where constructive feedback is received positively and regarded as a learning opportunity
- Display creativity and innovation in exploring and implementing possible solutions to problems and evaluating their effectiveness
- Contribute to the supervision of undergraduate/pre-registration Health Care Professionals and support workers within the team
- To be able to sustain postures such as kneeling, sitting on the floor and crawling during treatment sessions.
- To frequently manoeuvre, “set up” and adjust items of equipment following Health and Safety risk assessment protocols.
- To complete moving and handling assessments and demonstrate to carers, this may include hoisting or physical handling of patients.
