Job summary

We are looking for a person to lead on delivering a positive discharge experience for patients, families, carers and staff, whilst ensuring that the transition from hospital to community is safe and timely. You will be required to lead and influence across the Heath and Social Care System at a Senior Level.

This exciting role gives a unique opportunity to work with expert clinical and operational teams within South Tyneside and Sunderland, as well as within our Transfer of Care Hub and at wider System level, to ensure we deliver effective, safe and excellent care to our patients.

The post holder will have knowledge of large teams, in either acute, community or social care settings and have a proven track record of delivering meaningful and sustained change. The successful candidate will possess a strong ability to build successful teams whilst supporting, influencing and motivating staff to deliver challenging targets through times of change. The post holder is expected to provide proactive and visible leadership across the discharge systems and we are looking for someone who is ambitious, focused and passionate about patient care, shaping the challenge and opportunities across multiple partner organisations. You will be well supported by an experienced leadership team and have opportunities for personal development.

Main duties of the job

  • Strategically lead on the development and implementation of systems and processes to ensure effective flow of patients between hospital and community settings, using a Home First and Discharge to Assess Model.
  • Ensure best use of all available community capacity and resolve blockages to timely discharge through effective co-ordination of resources across all partner agencies, to ensure a timely and integrated health and care response.
  • Be instrumental in working with partners to ensure capacity matches demand.
  • Be a senior manager who can make decisions on behalf of all agencies in the system to facilitate discharge; to connect and be seen as working for the interest of the system as a whole.
  • Support key operational staff from partner organisations, who will be accountable to the post holder for the delivery of work whilst working within the TOCH at both Sunderland and South Tyneside Hospital sites, to ensure the national Hospital Discharge and Community Support: Policy and Operating Guidance is implemented, monitored and evaluated locally for the system.
  • Build strong networks and alliances to deliver change and act assertively to challenge lack of progress and resolve conflict.
  • Strategically lead the work required around capacity and demand modelling, for bed-based rehabilitation and community services related to discharge to assess, to ensure good flow out of hospital. Promoting a Home First approach.
  • Represent TOCH position at System Wide Surge.
