Job overview

We are looking for a band 5 nurse to support and advise wards to undertake timely planning and actions related to patient discharge.

Your role will be to provide all information relating to discharge and manage patient and family expectations , minimising the amount of time patients need to stay in hospital.

This is a full-time position, however, part-time hours (minimum of 30 hours) maybe considered.

Main duties of the job

You will support ward managers and clinicians to facilitate effective co ordination of appropriate interventions and services that meet the complex discharge requirements of patients.

Ensuring safe and affective transfers of care form acute setting to community /home setting.

Acting as a patient advocate ensuring that the patient family and carers are fully informed and supported at all times.

Supporting the acute ward teams in early identification of all patients with complex needs who will require efficient and effective discharge planning and coordination in order to prevent delays in transfer of care ,

Supporting sustain and embed the SAFER methodology

You will be an active participant  on daily board rounds  and to ensure that all patients have a meaningful estimated day of discharge.

Ensuring that all data is captured live on the Frontier system

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
