As an occupational therapist working within the Adult Community Therapy team, you will have the opportunity to champion the key values of your profession.

We support one another to meet the needs of patients living in Lincoln, Gainsborough and all the villages in between.

Your caseload will be varied: you will have the opportunity to work alongside adults experiencing a range of long-term health conditions and acute deteriorations in their functional abilities. A key aspect of this will involve assessment of frailty and providing meaningful support for people with palliative health conditions.

We work closely with the wider multidisciplinary and multiagency teams to ensure that our patients receive high quality and equitable care.

Main duties of the job

  • To assess and set goals for patients living in their own homes and care settings
  • To provide high quality occupational therapy interventions
  • To provide clinical leadership and supervision to members of the therapy team
  • To represent Adult Community Therapy in the context of the wider multidisciplinary and multiagency team
  • To work alongside the therapy leads in order to innovate and develop the therapy service
  • To demonstrate a commitment to personal development.
