Job overview

We seek dynamic nurses with amazing interpersonal skills to support the leads in the county wide Community Rapid Response & Intermediate Care Team, working to get people out of hospital or preventing hospital admission by playing a major part in their journey, setting goals and support planning with Therapists, Doctors and support workers as an integrated approach for patients in their own home environments. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments, including IV therapy and all aspects of nursing care in the patient’s own home environment.

The Community Rapid Response & Intermediate Care Team operates as a Virtual Ward across the county, managing patients with sub-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.

Main duties of the job

In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patients’ needs as and when required. The shifts for the service are between 07.00am to 01.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns. We can offer long and short shifts.

Applicants must hold a valid UK driving licence, have access to a vehicle for work purposes at all times on duty and be confident to drive in all driving conditions for this community post. Applicants will be flexible and adaptable, being able to respond to need at short notice.

To be considered you must have:-

  • Excellent organisational ability
  • Senior nurses band 6/7 excellent leadership skills
  • Superb level of communication skills
  • Able to flex to a changing environment for service improvement
  • Good working knowledge of digital communication products
  • Real interest in improving & delivering healthcare in the community.
  • Clinical triage skills- desirable
  • High level assessment, diagnostic and nursing clinical skills-desirable
  • Prescribing-desirable not essential
  • Be capable of working under pressure across a variety of nursing treatments
