HPFT’s Specialist Learning Disability Services are looking for a highly motivated, compassionate, and experienced Senior Community learning disabilities nurse  to work across Buckinghamshire leading the work of the nursing team.

The diverse and multi-disciplinary Specialist Learning Disability Services provide specialist care to adults with Learning Disabilities whose needs cannot be met by mainstream services alone. The successful candidate will be based at our Hub at  Midshires Business Park in Aylesbury or Anglo Office Park in High Wycombe on personal choice.

Under the direction of the Team Leader, you will provide comprehensive professional and effective leadership, as well as clinical advice and expertise. Working hours Monday to Friday 9am to 5pm.

We are seeking applications from Nurses who have positive attitudes, values and beliefs about working with people with an LD who are experiencing mental health problems, Positive behaviour support plans  and who want to make a real difference to the service.

Detailed job description and main responsibilities

In your new role you will:

  • Be responsible for supporting service users with a learning disability and health need in the community.
  • Carry out assessment of individual needs, make recommendations of how to meet those needs, support the implementation of these recommendations and monitor, evaluate and review the input
  • Work in close liaison with Team Manager, Nurse Team Leader, and Clinical Leads in delivering effective evidence-based practice to adults with Learning Disabilities requiring mental health services
  • Take a lead in and share the responsibility for identifying and developing opportunities for Service Users with learning disabilities to be enabled to comment on local and Trust wide service operational and development.
