An exciting opportunity has arisen for a dynamic, forward thinking, self-motivated individual to join our Community Response Team based in Stratford.
The Community Response Team operates across the hours of 08:30 to 21:30, 7 days a week, including weekends and bank holidays.
As a Senior Community nurse you will be responsible for assessing and managing the health and care needs of patients in their homes, aiming to prevent unnecessary hospital admissions and supporting to facilitate discharges from hospital. You will be supporting patients in the community with long term conditions, providing end of their life support and providing holistic, comprehensive care to patients in their own homes. You will also work closely with the Multidisciplinary team including: Therapists, GPs, Social Services, Urgent Response , working collaboratively with the other community service teams to deliver seamless care across the locality, working within an unpredictable and unscheduled workload.
You will also be responsible for deputising on behalf of the Band 7 professional Lead and providing coordination for the team. You will have proven organisational and management skills and will be able to motivate and effectively lead the teams in delivering high quality care to our patients. You will be a clinical role model and have a willingness to develop the team as well as your own professional growth.
Working in a leadership role as a member of the community response team, responding to visits via Clinical Triage assessing, planning, coordinating, delivering and evaluating the programmes of care provided across health, social and voluntary care sectors as part of a multi-disciplinary community-based rehabilitation team. Within the community, client’s home, residential homes and other sites as the service requires.
