The Carer Liaison service in Adult Specialist Services is expanding, following successful implementation of band 3 Carer Liaison roles at Ravenswood and Southfield and the Triangle of Care initiative.

With increasing project work and opportunities for additional staff responsibilities, the service now requires a structure to enable shared workload and future progression.

Main duties of the job

*To support the development of staff (Band 3) in relation to service user and carer engagement, through induction, training and supervision structures, where appropriate.*To initiate, establish and maintain relationships and support Carers, in managing their carer role.*To carry a caseload of carers and maintain good communication throughout the pathway of admission to discharge*Provide a range of opportunities which enable carers to gain information such as written, verbal and virtual data.*To work with internal and external agencies such as the Patient and Public Engagement Team, Carer Peer Support Workers, Complaints Team, community services and Trust Stakeholders*To create, maintain and encourage opportunities to have the carer's voice heard, such as forums, questionnaires, focus groups and research opportunities.*To be a point of liaison between carer and staff, incorporating the engagement of the service user at, or as close as possible to, admission.*To take a lead on creating, adapting and delivering training to all staff to improve carer awareness*To promote Triangle of Care (ToC) training and stay up to date on ToC projects*To promote the carer liaison service across the wider service and in external settings, such as conferences and forums. Sharing example of good practice and learning from incidents.*To take on delegation from the Service User and Carer Engagement Lead in their absence, chairing meetings, training and events.
