Homerton Healthcare are looking to appoint an Advanced Clinical Practitioner in Same Day Emergency Care (SDEC). This post is based at the Homerton Hospital in Hackney, East London, an exciting and challenging place with a diverse and richly multicultural population.

Same Day Emergency Care is a relatively new model of delivering care within the Urgent and Emergency system aiming to convert unscheduled care to that which can be delivered on the same day, as an alternative to prolonged Emergency Department (ED) attendance or hospital admission. This model is based on the premise that a significant proportion of patients requiring emergency care can be safely managed without an admission and without compromising quality. The Homerton SDEC unit receives referrals from the ED, general practice (GP), NHS 111, the London Ambulance Service as well as numerous other community providers. As well as providing treatment and investigation for new referrals the SDEC unit also facilitates safe early ward discharges reducing hospital length of stay.

The unit is led by General Medicine and Emergency Medicine Consultants and has 10 treatment areas with appropriate monitoring and resources to support a wide range of acute medical and surgical presentations. The unit operates a full medical service on weekdays between 08:00 and 20:00 and a limited nurse-led service at the weekends between 10:00 and 14:00.

This role provides an exciting opportunity to work closely with highly skilled and knowledgeable consultants with opportunities to work in a service where quality improvement and iterative change are happening in a real-time but supported way. A wide range of pathology will be seen in the day to day working of the unit, providing experiences that are both challenging and varied.

The post holder will have dedicated time to pursue other professional development and management activities. This may include quality improvement, clinical informatics, education, and point-of-care ultrasound. This time will be incorporated into your pattern.
