We are looking for a Band 6 Senior Mental Health Assessment & Brief Intervention Practitioner to join the Teign Core Adult Community Team.
This is a really great opportunity to join the team at an interesting and exciting time as we work towards transforming our community service so that we offer the best and most comprehensive care to the people of South Devon.
There will also be oportunity to work within the PCN to develop closer links with our Primary Care collegues

The team promotes a dynamic and innovative way of working that includes triage, signposting and conducting mental health assessments.
Working as dedicated practitioner to primary care network (PCN) as part of PCN multiagency team alongside mental health treatment practitioners, psychology, psychiatry and social care.
