We have an exciting development opportunity for a Pharmacy Technician to join our new Hospital@Home pharmacy team. You will work closely with the newly appointment Hospital@Home Senior Clinical Pharmacist implement this new service. The service also includes a frailty same day emergency care unit within the hospital.

The role will be based at University Hospital North Durham and the service will run Monday to Friday during core working hours. There is no weekend working commitment asscociated with the role.

You will belong to a team that is committed to supporting staff health and well-being as well as professional developemnt in order to help you achieve your full potential.

You must be registered with the General Pharmaceutical Council (GPhC) and hold a L3 Diploma in the Principles and Practice for Pharmacy Technicians or an equivalent, recognised GPhC qualification.

Main duties of the job

The post holders will as part of a multi-disciplinary team provide medicines support to the Hospital@Home team. The team includes Consultant Geriatricians, Advanced Nurse Practitioners and Therapists.

You will support the Hospital@Home pharmacists in delivering medicines optimisation to patients to promote the seamless transfer of care, medicines reconciliation and work with patients and their carers to promote independence with medicines. There may be a requirement to visit patients in their own homes to provide pharmacy support and advice

As part of the pharmacy team you will be involved in the day to day running of the ward, support training on medicines and be involved in audit and governance.

This is an exciting opportunity to shape a service that will improve the care of frailty patients in our local population and deliver an NHS national priority. You will develop the role of the pharmacy technician in the service for the benenfit of the patient.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
