We are seeking a dynamic and committed Registered Children’s Nurse with a special interest in children and young peoples  diabetes to join our current Transition Diabetes team . This role would be to support our Diabetes Transition Service for  our young patients aged 16-19yrs  with both Type 1 and Type 2  Diabetes.

The  CYP Diabetes service is for children and young people up to 16 years of age, with our transition service supporting up to 19 years. Our Transition  diabetes team  currently has one band 7 nurse. This role will be to work alongside the Transition Lead Nurse  and the MDT.  Although this role is primarily for the Transition  Diabetes service, there will be some requirement to cross cover and work together with the Paediatrics diabetes services and MDT to provide a first class service to children, young people and their families.

Main duties of the job

We currently have approximately 85 patients aged 16-19yrs  with diabetes who we provide a service to, with approximately  35  receiving insulin pump therapy. You will work within the paediatric and adult outpatient setting, inpatient ward setting and also in the community  to provide a high quality service to  young people with diabetes and their families.

You will have post registered experience of working with children and young people with diabetes in a hospital or outpatient setting, and be interested in developing your skills and role further.

You will need to be flexible in your approach to work and in the hours that you may be required to work in the clinic and community in order that we can provide a service to children, young people and their families. The CYP Diabetes team currently provide an on call service at the weekend, which you will be expected to take part in.

Detailed job description and main responsibilities

Specific Responsibilities Clinical responsibilities:

Responsible for the assessment of the care needs of babies, children and adolescents, and develops programmes of care in relation to the needs of the child, young people and family in relation to diabetes.

Responsible for the implementation of programmes of nursing care and evaluation of their effectiveness, ensuring that the care plan provides a continuous record of progress, response to treatment, rehabilitation and nursing care. responsibility for a defined caseload of diabetic patients within the inpatient/outpatient setting under the supervision of the Band 7 Paediatric Diabetes Nurse Specialists.

Work in partnership with children, young people and their families in order to support and encourage involvement in their care, thus supporting family centred care. To also encourage independence and self care for our 16-19yr olds.

Takes the lead in supporting parents, carers, siblings and other members of the child/ young person's support network as appropriate.

Responsible for, and takes the lead in setting and maintaining optimum standards of nursing care, ensuring that nursing guidelines and policies are adhered to within the inpatient/outpatient setting in relation to diabetes.

Work without direct supervision and be responsible for the management of his/her caseload within the hospital and outpatient setting under the supervision of the Band 7 Paediatric Diabetes Nurse Specialists.

Maintain effective communication with multidisciplinary team members, other hospital nursing and medical staff, general practitioners, community nursing staff and statutory and non statutory organisations that may be involved with the care of the child and family.
