This is an exciting opportunity for you to come and join our friendly team.

We are a multi-agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events. We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex.

This will be a band 5 - 6 development post. This is a great role for a nurse with an enthusiasm for Health Promotion and a good knowledge of local health and social care. A band 5 nurse who wants to further develop their skills in early health intervention. It's a job that comes with flexibility and the scope for you to bring in your ideas on how to reduce health inequalities across West Sussex.

The post will be based at Glebelands in Shoreham but the successful candidate  will be expected to work across county to support the needs of the service.

Main duties of the job

  • To deliver NHS Health Checks to the public in outreach venues after appropriate training. This requires a holistic approach assisting people to access services designed to improve their health & wellbeing through behaviour change. This is part of our duties as providers to West Sussex County Council’s Public Health Department.
  • To work closely with other PAT colleagues, primary care teams, social & caring services and the voluntary sector in the development and delivery of integrated services to avoid duplication and overlap.
  • To contribute to covering the Duty Line along with other PAT colleagues.
  • To contribute to scoping local services/initiatives and, along with other PAT colleagues, contribute to the maintenance of a database/directory to ensure accurate signposting and outcomes.
  • With reference to routine data and other information available from West Sussex County Council’s Public Health Department and other sources, ensure that services and resources are targeted towards those populations where need is the greatest, thereby tackling health inequalities. This is carried out with guidance from the multi-agency PAT Board.
  • The post holder is not responsible for leading or managing other members of the team however, will occasionally be responsible for pre-registration student nurses who are on a community placement within the PATs i.e. student shadows NHS Nurse Advisor on a visit or when carrying out an NHS Health Check

Detailed job description and main responsibilities

Provide a holistic assessment of health, social and environmental needs of adults and older people living in the community.

Provide advice, signposting and onward referral to a range of statutory, voluntary and community services to enhance the health and wellbeing of adults and older people by utilising up to date knowledge of relevant national & local policy developments.

Work as part of a multi-agency team which includes health, social and caring services and voluntary sector staff;

Attending regular PAT & NHS Nurse Advisor team meetings, sharing lessons & supporting the development of new services.

In conjunction with other multi-agency colleagues continue to develop, expand and implement a health enhancing, population based approach to improving the health and wellbeing of adults and older people in the community.

Using a robust evidence based approach, working with the Wellbeing Hubs, actively plan and implement activities designed to promote independence and wellbeing amongst adults and older people living in the community.

Keep up to date and contemporaneous records adhering to National and Trust policies and procedures.

Demonstrate commitment to continual professional development, participating in clinical supervision and working within the NMC Code and Sussex Community NHS Foundation Trust policies.

Attend all statutory and mandatory training and any other training required for the role.

Deliver NHS Health Checks in outreach venues.
