We are delighted to offer an exciting opportunity to join our Community Nursing Team as a Lead Practitioner in our CIS team.
We are looking for NMC registered professionals who can provide strong clinical leadership and support safe and effective care for service users to join our busy team based in Seisdon.
Our team work within a new service delivery model and we would like you to lead in the development of the role to further support our teams. You will work with the Operational Team and other senior nurses to share responsibility for organising the day-to-day running of the caseload, managing emerging risks, supporting effective reporting and governance and agreeing service transformation priorities.
You will to be highly motivated and enthusiastic, have excellent communication, negotiation and interpersonal skills and an ability to challenge practice as needed. You will be able to demonstrate competence in core community nursing clinical skills and a willingness to learn and share knowledge.
You will utilise your enhanced assessment skills to support decision-making and be competent and confident across areas of practice and provide enhanced holistic nursing care to housebound service users utilising enhanced assessment skills; decision making; prescribing and freedom to act as a Specialist Practitioner.
As part of this role you will be expected to:
Formulate plans of care and negotiate the implementation of such plans and the sharing of complex, sensitive, confidential, and at times contentious information e.g. explaining diagnosis and treatment plans; discussing disease progression and prognosis.
Support staff development in the service by providing mentorship and supervision to pre and post registered nurses.
