Job overview

An exciting opportunity has arisen to join our dedicated community heart failure team in Brent as a Band 7 community heart failure specialist nurse on a part-time basis.

In this crucial role, the Heart Failure Specialist Nurse will work as an autonomous clinical practitioner, providing a high-quality community heart failure service to a specific caseload of patients, enhancing their quality of life, promoting health excellence and independence within the community, preventing unnecessary hospital admissions, and providing education and health promotion.

We look forward to recruiting an individual who provides a high standard of evidence-based care and who is committed to continually improving the service provided for heart failure patients.

The successful candidate will be expected and supported to complete the physical assessment of the adult and non-medical prescribing qualifications.

You will be an experienced cardiology nurse with relevant post-registration training.

You will have supervisory responsibility and be able to guide and teach members of the clinical team.

Main duties of the job

A flexible approach to practice, excellent communication skills, the ability to work autonomously and a qualified driver are all essential for this post.

The service provides complex care to patients with heart failure within the community setting.

The post-holder will be joining a multi-disciplinary team working with other Heart Failure Specialist Nurses, Cardiologists, and the wider Brent Integrated service.

The position involves working closely with the primary and secondary care teams, to provide specialist heart failure care and expertise to patients diagnosed with heart failure, and to ensure that patients receive seamless care in hospital, at home and in the outpatient setting.

As well as the clinical delivery, you will be required to lead on various service development projects such as audits, patient stories, and patient engagement. In addition, you will be contributing to up-skilling other health care professionals to provide a high standard of care.
