Job overview

The post holder will work within the established cardiovascular rehabilitation team to deliver and support high quality cardiovascular rehabilitation programmes for patients with a wide range of cardiovascular conditions through individualised patient assessments, formal cardiac rehabilitation exercise classes and 1:1 exercise programmes.

The post holder will support the standards of clinical practice, in conjunction with the multidisciplinary team. Also, must have the ability to travel across and deliver cardiovascular rehabilitation across South Cumbria.

We have 1 x full-time positions available.

Main duties of the job

Managing own patient caseload, including the preparation and completion of patient assessments to establish physical, psychological and educational needs and goal setting plans including families and carers and being responsive to any communication barriers.

• To work within standards and protocols set by the BACPR and ACPICR
• Being responsible for the supervision and demonstration of exercise-based cardiac rehabilitation sessions.
• To deliver education talks as part of the cardiac rehabilitation programme
• Ensuring adequate levels of supervision of all rehabilitation sessions in line with service policies and procedures and according to the patient’s care plan.
• Ensuring that all patients progress through the relevant stages of the programme at the correct time and that appropriate information and advice is provided at each stage
• Assuming responsibility for monitoring the progress of all patients undergoing cardiac rehabilitation, evaluating their condition and adapting their programme as necessary within department standards and protocols, seeking senior assistance as required.
• Maintaining all records associated with cardiac rehabilitation in an organised and confidential manner in accordance with the Code of Conduct and Caldicott principles.
