As Health Visitors our role is to lead on the Healthy Child Programme and to prevent ill health. Health Visitors work closely with families to achieve positive outcomes and they take responsibility for the ensuring that the physical and emotional needs of the family are met.
The post holder will have responsibility for a defined community which will include assessing, implementing and evaluating care offered the the families.
You will be expected to provide a universal service across the caseload and you will have a high degree of autonomy and will be able to provide specialist knowledge and advice and take responsibility for coordinating and delegating work within the skill mix team.
Consideration will be given to student Health Visitors who are due to qualify or who are newly qualified as a preceptorship offer will be available.
Provide and maintain a high standard of care adapted to meet the needs of a defined community, ensuring that service users receive an appropriate service jointly agreed with the Health Visiting Team.
Provide a high level of specialist care, assessment and evaluation to families, children in need and children in need of protection, maintaining the standards set at local and national level.
Negotiate individual healthcare plans with the service users; continually assessing and evaluating to ensure that agreed outcomes are adequately met. Assess and prioritise specific health needs and facilitate group work where appropriate.
Where appropriate liaise with members of the multi-agency team in planning, delivering and evaluating care with specific aims of improving long term outcomes for vulnerable groups.
Initiate interventions when working with clients such as motivational and promotional interviewing.
