Due to staff progression we currently have various Health Visitor hours available within our Health Visiting service based within Stratford.
Southam Health Visiting offer a wide range of Health Visiting experience and opportunities. You will find our Warwickshire Health Visiting service a welcoming and supportive place to work, and a supportive preceptorship programme exists for newly qualified Health Visitors.
We are looking for Health Visitors who are proactive, innovative, like a challenge and enjoy working as part of a lovely team. Whilst the post is based at the Stratford Hospital, we promote agile working and will consider flexible working requests.
Car driver/owner essential.
To effectively lead and deliver on the Healthy Child Programme (HCP) 0-5 (DH 2010) to improve outcomes for the children and families.
Named responsibility and professional accountability for the management of a defined team caseload including the delegation of tasks to members of the team.
To contribute to the Public Health Agenda, meet Public Health targets and reduce health inequalities.
To actively lead teams both within Health Visiting and partner agencies.
To work as part of a team to ensure that the services key performance indicators targets are achieved.
To act as a role model and be professional at all times adhering to the current NMC Code of Conduct.
To participate in nurse prescribing following NMC and local policy.
Act as a named Health Visitor for a defined team caseload.
To develop a team caseload profile, assessing needs as Universal, Targeted and Specialist.
Develop and implement strategies to improve Public Health through Health Education, Health Promotion activities using evidence based research and practice. Leading and delivering on the Healthy Child programme 0-5, working with partner agencies to improve outcomes for children.
Plan, implement and evaluate health enhancing activities through home visiting, community development, clinic, workshops, technology and parenting support.
Early identification of children and families needing support through use of single assessment process if required to include other members of the multidisciplinary team.
Act in accordance with current local Warwickshire Safeguarding Board and Trust policy to protect children at risk.
Delegate and supervise work undertaken by the skill mix team.
Promote and maintain good record keeping that conforms to NMC policy, data protection and Health Visitor record keeping standards.
Identify and report any hazards/ incidents relevant to the Health and safety of clients or colleagues using the Trusts reporting procedure.
Adhere to the Trust lone worker policy and undertake risk assessments where needed in liaison with Clinical Lead.
Participate in selection and recruitment of new staff to the team.
To facilitate and organise the induction of new Health Visitors and support staff to the Trust.
To escalate any concerns as appropriate to your Clinical Lead.
Knowledge, Skills and Experience
To be professionally and legally accountable for all aspects of own work ensuring a high standard of clinical care. To provide effective clinical leadership to the Health Visiting team, enabling the team to deliver the highest possible care standards ensuring evidence based practice is used. Adhering to the Health Visiting Standards, and Standard Operating Procedures.
Demonstrate a good knowledge and understanding of relevant legislation, policies and procedures relating to child protection. Participate in risk assessment & decision making process at child protection meetings, providing written reports, and health advice when required.
To actively participate in supervision, Child Protection, Clinical/ Restorative and Looked After Children.
To prescribe using the Nurse Formulary to ensure prescribing is undertaken in a safe, cost effective manner.
Be flexible to the needs of the service.
Develop and maintain effective communication within the team to maintain productive working relationships and promote good morale within the team resulting in a quality service.
Ensure that all clients and colleagues are treated at all times in an equitable manner, respecting and showing an understanding of diversity in the workplace.
Ensure confidentiality is upheld at all times, as per Trust guidelines.
Attend the Trust mandatory and statutory training programme required for your role ensuring that you training is completed and follows the essential skills policy including regular appraisals. Ensure skill mix team members are up to date with their statutory and mandatory training including appraisals.
Participate in and contribute to team/ locality meetings to keep up to date with Trust information.
To be competent using Microsoft and other computer / phone applications as required.

To input patient contacts onto Trust computer system.
To ensure electronic work diary is up to date and all members of the team have full access to it.
To maintain mentorship and mentor pre-registration nursing students.
