To take a Team Lead role in the provision of services within the General Medicine Directorate supervising the Inpatient Medical Occupational Therapy Team on a day to day basis in co-operation with the Head and Deputy Head of Occupational Therapist.
To ensure a high standard of Occupational Therapy service within the Inpatient therapy team specifically in services to General Medicine, Cardiac Care, Respiratory Care, Oncology and Palliative care.
To provide comprehensive OT assessments, interventions and advice for patients and relatives, using evidence based client-centred principles.
To enable provision of Occupational Therapy services in the prevention of admission and facilitation of discharge in, and working closely with discharge planners and care managers to provide patient follow-up in the community facilitating integrated rehabilitation programmes, especially when patients move across professional and organisational boundaries.
To communicate effectively with
Patients, carers and other staff imparting complex and sensitive information and unwelcome news.
With Senior Occupational Therapy staff to maintain a coordinated approach to service delivery and development.
With qualified Occupational Therapy staff, Technical Instructors, Occupational Therapy Assistants and students on clinical placement to support team working on a day to day basis
With medical, nursing, paramedical, and other hospital staff to promote a co-ordinated approach to patient care and multi-disciplinary team working in the preparation and implementation of treatment/care policies, procedure and care pathways
With all disciplines in the community, particularly other Occupational Therapists, to provide continuity of care and promotion of integrated care across the Acute Health Services Unit, Social Services, Community Care, and Carers
To contribute to and execute the Patient Discharge Care Plan through effective communication with the patient, family and carers
With appropriate disciplines in other Trusts as required, to promote patient care, service objectives
To be professionally and legally accountable for all aspects of own work, including the management of patients in your care. To ensure a high standard of clinical care for the patients under your management, and support more junior staff to do likewise
To hold responsibility for own case load and be responsible for a defined area of the service or a particular patient type, working with out direct supervision.
Supervision takes the form of regular formal training and clinical reasoning sessions, peer review, case conferences. Access to advice and support from a senior Occupational Therapist is available if required, clinical work is not routinely evaluated
To undertake all aspects of clinical duties as an autonomous practitioner
To represent the Trust externally regarding the Occupational Therapy Inpatient services provided to patients in the General Medicine Directorate as delegated by the Head and Deputy Head Occupational Therapist or the Head of Therapies.
To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi-pathologies; use advanced clinical reasoning skills and assessment techniques to provide an accurate diagnosis of their condition.
Formulate and deliver an individual Occupational Therapy treatment programme based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills e.g. Occupational Therapy treatment techniques, cognitive assessment, patient education, exercise classes and other alternative options.
