Job overview

This post is to support and facilitate patients who have a positive FIT test and or are referred on a 2 week wait fast track referral for a suspicion of a gastrointestinal cancer.

Working with the MDT Clinical and Admin staff to ensure patients are treated within a timely manner and to improve the 28 Faster Diagnosis Standard.

We are looking for a highly motivated individual who has NHS  experience to facilitate and navigate this  complex cohort of patients pathways. The need to work autonomously with minimum support following intensive guidance and induction is paramount within this role.

Main duties of the job

Monitor all patients  with a positive FIT test result and navigate all patients  referred on a gastrointestinal suspicion of cancer pathway from point of referral to support them to  attend all appointments and diagnostics within their pathway.

Instruct and monitor patients on undergoing FIT diagnostics and chase results.

Support service improvement and undertake audits with support to track processes and improve outcomes

Cover LGI/UGI MDT Pathway  Coordinator to facilitate and manage MDT  weekly  meetings.

Liaise with patients/families/clinical and admin staff  within  Primary Care and secondary Care  to ensure patients are treated within a timely manner in line with the Bradford and Airedale FIT Pathway Guidance. (Adapted from West Yorkshire and Harrogate Cancer Alliance FIT Pathway)

Support and sign post patients with a positive FIT test

Support and guide patients to complete FIT test

Complete, monitor  and facilitate FIT data Dashboard

Support MDT Admin and clinical staff to help improve Cancer Wait time targets and patient experience for patients on a  suspicion of cancer fast track pathway.

Help support and improve patient experience

Help support and improve service issues and streamlining of pathways

Cover and support MDT Pathway Coordinators within their role.

Attend education events regarding gastrointestinal cancers/FIT pathways
