The Wokingham Community Older Adults Mental Health Team is looking for an enthusiastic, caring and highly motivated Mental Health Practitioner (Mental Health Nurse, Social Worker, Occupational Therapist) to join our friendly and innovative Community Team

The Wokingham Older Adult Mental Health Service is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will take responsibility for the assessment and implementation of intensive care and treatment in the homes of people with functional mental health problems, as well as people with dementia.

This is a developmental role- the successful candidate will join the team as a B5 Mental Health Practitioner and progress to a B6 following a period of 18 months in supported practice and successful demonstration of competence.

The successful candidate will have the relevant clinical experience and must display a passion and understanding for working with Older People, appropriate training and support will be provided as needed.

You must be enthusiastic, caring and highly motivated to contribute towards maintaining standards of high-quality care to our patients. In our latest CQC inspection we achieved ‘Outstanding’ and therefore the successful candidate would be expected to work towards helping the service maintain these accolades of high standards

Main duties of the job

  • The successful candidate will be responsible for managing their own case load, monitoring the mental health needs of patients and working alongside their carers to provide education and support. We are currently trialling a new model where health staff are not care coordinating. (This may change in 2025 as we review the trial). As it stands you will be identifying care needs under the Care Act and referring to our social work team. Knowledge of the appropriate legislation such as the Care Act, Mental Capacity Act and Mental Health Act is key.
  • To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team.
  • To provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community such as Residential Homes, Sheltered Housing, Day Centres etc.
  • To have knowledge of therapeutic interventions such as crisis intervention and problem solving

  • You must hold one of the below relevant and current professional qualification - Social Worker, Mental Health Nurse, Occupational Therapist
  • You must have the relevant clinical experience and a passion and understanding for working with Older People
  • You must hold a valid UK driving license with the ability to travel to a variety of settings.
