Job overview

Trinity Ward is a 25-bed Nurse Led Community Hospital ward.

Trinity Ward is mainly an Elderly Care ward providing rehabilitation beds following trauma/surgery and non-acute medical/ minor illnesses.

We also provide palliative care beds for those reaching the end of life or in crisis at home. We provide dignified holistic care for the patient and their families in a calm and caring environment.

We promote patient involvement and empower patients and family members to provide input into their clinical care and decisions.

We respect that each individual has the right to autonomy and independence and endeavour to promote that independence as able.

An excellent opportunity has arisen for a motivated, dedicated individual to join this established and hardworking team.

You will holder will be responsible for the delivery of the highest quality of care to a diverse client group predominantly managed within a nurse led facility. You will utilise highly developed clinical skills including diagnostic skills, alongside experience of long term condition management and elderly care.

The role requires you to be able to communicate well and have the required leadership skills to deal with a MDT team and assist in case managing a group of patients from admission to discharge.

Main duties of the job

Provide and support the team to provide quality  patient focussed care.

Provide holistic care for patients and relatives.

Administer medication, assess and implement care plans.

Act as a resource for the  multidisciplinary integrated team, carrying out assessments, and contributing to the development and delivery of the care. Provide support, training and advice to patients, parents and carers. Manage the performance and oversee the work of more junior staff, assistants, students and volunteers Manage the work of the team, deputising for more senior managers as required.

You will contribute to ESNEFT’s overall patient care delivery model ensuring high quality, effective care for all.

Provide care to patients on a frequent basis, with the ability to deal with distressing and emotional issues and unpleasant conditions as and when required Be aware of and able to deal with untoward incidences e.g. physical or verbal aggression.

You will holder will be expected to participate in audits and investigations, and relay learning to the rest of the team.

Be able to prioritise your caseload and that of your team. Ensuring patient care is prioritised, patient needs are escalated and documentation of that care is completed
