Job overview

We have an exciting opportunity for an Integrated Community Support Worker to join our Community Nursing Team to work across our Cannock clinics.  We are looking to recruit dedicated and compassionate staff who strive to offer the high quality patient experience we offer in the community.

The role will require you to work alongside our qualified staff who undertake wound care and catheter care across our clinic locations. The role involves clinical tasks as well as supporting the team with stock and patient management.

This is an exciting time to join our team and you will be a part of excellent way of working to meet the changing demand on community nursing.

It is important that you can complete patient records accurately after each visit and be confident in assessing any changes in the patient’s health, communicating these changes back to the health professionals involved in their care plan.

You will need to be able to demonstrate your previous experience of working in a caring role in either a formal or informal care setting and be able to bring this experience with you.

As this is a community-based post it is essential that you can travel to various locations across the geographical area of the service, therefore you must be a driver and have access to a vehicle.

Main duties of the job

  • Be involved in the first point of contact for the discharge to assess pathway.
  • Assist in the delivery of outcome-based goals and self-directed goal planning
  • Undertake person centred support planning
  • Deliver support plans and safeguard vulnerable people
  • Contribute to outcome focused reviews
  • Encourage people's independence, choice, health & wellbeing
  • Identify the need and encourage individuals with the safe use of basis equipment and assistive technology, which may include in ordering basis equipment as required.
  • Facilitate individuals transition to alternative services and support if needed
  • Engage in organisational learning and development
  • Promote rehabilitation and reablement individual plans of care to meet patient outcomes
  • Appropriate recording and completion of all written and electronic records.
