Clinical Nurse Specialist Critical Care Outreach Team

Job overview

We are recruiting for a Clinical Nurse Specialist who share/s our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

The Critical Care Outreach Team provide support and expert clinical intervention to patients all around the Trust 24 hours a day, 365 days a year. We advise, intervene and advocate for any patient who has a need relating to deterioration, we support staff and manage patients when they step down from Critical Care to ward level.

We are looking for someone with advanced clinical assessment skills, high levels of empathy and compassion and the ability to the patient at the heart of all they do.

Main duties of the job

Provide expert clinical assessment for all and any patients showing signs of deterioration

Provide oversight of patients with laryngectomies and tracheostomies in ward areas

Deliver assessment and be a compassionate advocate for patients under Martha's Rule

Deliver training around the deteriorating patient both formally and ad hoc

Involvement in quality improvement projects, audit and data analysis

Participation in After Action Reviews and Patient Safety Incident Investigations
