Main duties of the job

The successful candidate will work the hours over 5 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is home visit / clinic  based. You will also work on the ward providing stage 0- 1  of the commissioning pack pathway & in community exercise classes.

  • Two   years post registration experience, 1 of which must be in recent CVD  care
  • Understanding of the Commissioning guide for Cardiac Rehabilitation
