Dudley Integrated Health and Care NHS Trust (DIHC) has developed and integrated primary care with community-based services within the Dudley healthcare system since 2020 with its core aim being community where possible and hospital when necessary. Our purpose is to improve the health and wellbeing of our local population in Dudley. We are currently working with organisations within the health care sector in Dudley and the Black Country System to look at the best way to continue to deliver the services to our patients. As such, services within the organisation will transfer to another NHS Trust over the course of the next 6 months.
The duties and responsibilities are intended to be indicative but not exhaustive of the responsibilities of the post holder. As Dudley Integrated Health and Care NHS Trust develops, the requirements of the job may develop and change.
However, to provide an insight into this role, the successful applicant could be expected to be involved the below:
• To support the procurement of care for CHC eligible individuals, following guidance on clinical needs, and in doing so deliver appropriately commissioned care placements and packages of care.• Communicate effectively with care providers to identify suitable CHC care packages and placements.• Maintain accurate information systems to evidence the effectiveness of the service• The post holder will be required to demonstrate initiative in organising their own workload as well as working independently. There will be an expectation for the post holder to be highly motivated, flexible, and enthusiastic.• To support the Brokerage team to identify suitable CHC care packages and placements. To develop effective communication systems and relationships with care providers.• Update systems to provide oversight of available care market capacity and commissioned activity.
The post holder will support the Bed Bureau and Continuing Healthcare Team in ensuring the people of Dudley experience the best possible quality of services, clinical outcomes, and patient experience when they interface with health and care providers.The Brokerage Support Officer will support the DIHC Brokerage Team in identifying appropriate placements for patients referred from DGH under bed bureau/Pathway 3 and from CHC team, as part of the role the brokerage support officer will be expected to manage the procurement and contracting for CHC packages of care.
