If so, this could be the opportunity for you! You will be part of a large multi-disciplinary team of 80+ staff supporting the transition of patient care from secure settings into the community, which feeds into various sites including Reaside, Ardenleigh, Tamarind, St Andrews and Hillis Lodge and Elliot House. We offer extensive support to our staff in their role: supervision, reflective practice, art therapy, autism support and case-busting sessions. We also provide flexible working patterns which includes working from home when needed. FIRST is a standalone service providing our own home treatment and crisis community support to our service users; our CPN’s and SW’s provide joint on-call cover with support from On Call RC (approx. 3-4 times per month) during evenings, weekends and bank holidays so that we can respond to urgent situations by telephone and complete home visits in emergencies (standby on call payments and overtime are payable).
Main duties of the job
The postholder will:
- Act as an independent and autonomous practitioner within a specialist community setting with the responsibility for intensive follow up of mentally disordered offenders.
- Manage a defined caseload, provide supervision to others as appropriate within the community and will be expected to have Full Driving License.
- Manage and assess risk of violence and offending behaviours whilst being mindful of our duty to public protection. Liaise with other professionals regarding risk management issues.
- Be responsible for the assessment, planning, implementation, and evaluation of nursing parameters and have a working knowledge of local, national policies and legislation which govern current service provision and audit.
- Be responsible for ensuring effective communication pathways exist.
- Be responsible for ensuring the promotion of equality, diversity, and rights in accordance with good practice and legislation.
- Be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate.
- Participate and contribute to the development of policy and strategy for community-based services within the Forensic Directorate.
- Participate in liaison with non-health care professionals i.e. Police, Probation and Courts.
- Participate in providing on call crisis intervention support during evenings, weekends and bank holidays on a rota basis.
