To work within the crisis hub alongside the Multi-disciplinary team to support the needs of the CYP who have a diagnosis of Autism/Learning disabilities who are at risk of admission to hospital.
To support with aspects of assessments, interventions and leasing under the guidance of the wider MDT
The successful applicant will work within the community and with the wider CAMHS service to contribute towards the prevention of hospital admissions and support recovery whilst advocating for the CYP and families.
Main duties of the job
- · To work effectively as a member of the team to engage young people in a variety of activities specific to their recovery.
- To respond to challenging behaviours as required and respond to behaviours of concerns and communicate with other staff members and escalate accordingly.
- To Advocate for Children and Young people and their parents/Carers as and when required.
- To contribute towards risk management planning by identifying and reporting incidents and risks to senior staff members.
- To participate in therapeutic observations with Children and Young People in a variety of settings, Under the guidance of an allocated clinician.
- To Work with the direction of the registered staff, Team Manager, Clinical Lead and Multidisciplinary team
- To carry out specific activities under the direction of the clinical staff.
- To work with individuals to establish meaningful activities.
- To work within the crisis Hub/M-power service with an aim to prevent unnecessary admission to hospital and support service users to remain within their home environment.
- To provide practical support to Children and Young People in developing and managing dignity and independence
- To provide information/signpost and encourage health promotion
- To record information and report to the Crisis/M-power team members, care coordinator on a regular basis via the MDT.
