The post holder will have responsibility for secretarial and administrative support to the Specialist Palliative Care Team (SPCT). The main role will be to support the facilitation of fast-track discharges for patients nearing the end of their life. The post holder will be responsible for coordinating the fast track care plans, working in collaboration with local Integrated Care Board (ICB) to commission care packages for home, care homes and Saint Francis Hospice to meet the patients’ needs.
