An exciting opportunity gas arisen to recruit a Band 3 Health Care Assistant with the Liver Transplant Assessment team. The Band 3 candidate should have previous experience within a health care setting including extensive phlebotomy skills.
The post holder will work as part of a specialist team looking after patients through their outpatient liver transplant assessment and onto their transplant journey on the waiting list and after liver transplantation.
The post holder will be required to work in the outpatient setting to ensure the provision of high class patient care and smoothing running of the transplant pathway.
You will be joining team of doctors, transplant nurses, dieticians and physiotherapists and will be an integral part of the team. We are looking for someone who works well in a team, has excellent interpersonal skills in all areas as you will be communicating with patients, relatives and staff, initiate and share ideas with the team and must be able to prioritise your workload.
Above all patients must be at the forefront of your practice
Assist the team with the delivery of the Liver transplant assessment programme service objectives. Work co-operatively as part of the multi-disciplinary team supporting colleagues in work associated with patient care. Initiate or recommend care as appropriate to individual patients in line with Trust Policy. Communicate clearly, effectively and appropriately with the multidisciplinary team, patients and their family, visitors and carers. Undertake audit activities as directed. Following appropriate training and supervised practice, undertake identified competency based practices / demonstrating and educating others including patients and their carers. Identify learning and development needs and produce a personal development plans in conjunction with your line manager / supervisor. Achieve and maintain competence in mandatory and statutory training associated with the role. Update and make records of interventions / training / patient care and treatment and report adverse incidents in accordance with Trust policy. Attend liver transplant service/ team meetings. Organise and prioritise own workload with direction and supervision of registered Nurses within the LRTC team. Work across all sites and premises associated with the Trust.
