Come and join our friendly, sociable, experienced teams working within the Joint Community Rehabilitation Service (JCR), Integrated Discharge Team (IDT) EDGH, Hospital Intervention Team (HIT) EDGH, and UCR (Urgent Care Response) EDGH.

The Joint Community Rehabilitation and Falls Prevention Service is a short-term 7 day service which assesses and treats people within their own homes. The team, uses an inter-professional model of working and aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life. We key work and provide rehabilitation to adults with a variety of medical and surgical conditions.  Weekends are covered on a rota basis.

The Hospital Intervention Team is part of a Multi-Disciplinary Team (MDT) and is based in the Emergency Department, the Clinical Decision Unit and the Acute Medical Unit. The aim of the service is to facilitate a timely discharge from these wards, and involves working as part of a 7 day rota.

The Integrated Discharge Team is part of a MDT and based on an acute ward. The service aims to assess clients on the ward and to make decisions regarding the help they require to return home (as per the National Guidance on Pathways – Discharge to Assess model).

Main duties of the job

To undertake holistic assessments, key work individual cases and provide specific Occupational Therapy intervention as required

To manage a clinical caseload, undertaking specialist clinical Occupational Therapy, including complex work requiring particular expertise or ability.

To participate as part of an integrated team; providing Occupational Therapy advice to other colleagues within the team

To support the Team Lead with service development, helping to implement policy changes and to set and monitor standards of practice.

To participate in health promotion and education of staff, clients and families
