Job overview

A wonderful opportunity has arisen to join the placement of care team as an Assistant Trainer. The role of the practitioner will be to ensure that the General Nursing assistants (GNAs/HCAs) are trained and  competent at completing a wide range of skills in community settings under direction from Lead Trainer.

Main duties of the job

The role of GNA is broad and they support a wide group of clients requiring multiple services ranging from rehabilitation within the home to end of life care. The focus of the service going forward is to enhance the skill set, focusing on a 'home first approach' from hospital, step up services from community and embedding a Cheshire wide End of life service for all of our palliative clients.

The post holder will be responsible from direction of the IPOCH Lead Trainer to support with the delivery of training to GNAs within clients' homes, supporting the training and induction of new starters. The role sits within the combined hospital and community discharge services and will be managed by the Lead Trainer under direction of Operational Managers.

Successful candidates must have a passion for training and ongoing staff development, excellent communication and IT skills and be able to work across a wide patch incorporating all of the  surrounding care communities. Knowledge and experience of working within community settings is preferred.  A full driving licence is essential.

Detailed job description and main responsibilities

SERVICE DELIVERY

  • Assess a wide range of clinical skills against an approved set of clinical competencies for the General Nursing Assistants working within the Integrated Placement of Care Hub.
  • Under the direction of the Senior Clinician provide training based on a pre-established training programme for the provision of individualised high-quality care based on a patient’s holistic needs.
  • Under the direction of the Senior Clinician support and facilitate the General Nursing Assistants experience to meet learning outcomes in practice.
  • Under the direction of the senior clinician within the team support the GNA induction programme.
  • Provide high quality direct patient care independently to patients within their own homes.
  • To support and wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
  • To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery.
  • To work within the agreed framework to assess, provide and review holistic care, advice and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge.
  • Assess, plan and implement and evaluate a plan of care to meet the patient’s physical, social, emotional and environmental needs, encompassing a rehabilitative/self-care approach where necessary.
  • To be able to recognise, interpret and analyse clinical data related to the individual clients needs including client observations.
  • Demonstrate, teach and supervise the care that can be safely delegated to the patient/carer giving support as necessary.
  • Promote clinical excellence and evidence-based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with competency framework.
  • Work to the End of Life (EOL) Strategy including adherence to all EOL tools, and have excellent knowledge and skill around EOL care provision.
  • Take an active role in health education of the individual and community promoting self-care and providing both planned and opportunistic care based upon evidenced based information.
  • Accurately record all patient contacts on EMIS web and Trust documentation as per MCHFT policy ensuring patient and professional confidentiality is maintained at all times in accordance with NMC/HCPC guidelines.
  • Manage and assess patient’s potential to be discharged from the caseload, including identifying any ongoing clinical or social needs.
  • Ensure that equipment and aids, necessary for effective care, meet the needs of individual patients and are adequately maintained. Demonstrate and explain safe use of equipment for patient and/or carer where required.
  • Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.
  • Refer people to other practitioners when needs and risks are beyond one’s own scope of practice or require longer term support.
  • To participate in the measurement and monitoring of quality standards as appropriate.
  • To maintain accurate personal and professional records adhering to MCHFT and policy and professional body guidelines.
  • To ensure the most efficient and cost-effective utilisation of all resources (human, physical and financial).
  • To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting system immediately. To participate in any investigations as necessary.
  • To contribute to and co-operate in the co-ordination of off duty and holiday leave with colleagues, ensuring service cover is maintained. Working effectively to support other team members in all aspects of the service.
  • To maintain all aspects of MCHFT Integrated Governance Strategy and requirements
  • To actively participate in appraisal and clinical supervision in accordance with Trust policy.
  • Act at all times in a manner which illustrates respect for privacy dignity and confidentiality

GOVERNANCE

Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.

MANAGERIAL/LEADERSHIP

  • Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.

EDUCATION/LEARNING

  • To assist the Lead Trainer with the planning and provision of training for General Nursing Assistants to demonstrate the application of theory to practice within the Community.
  • To assist the Lead Trainer with the orientation and agreed induction programmes for newly appointed staff, and those requiring Community experiences.
  • To assist the Lead Trainer with training of other Team members and staff as appropriate.
  • To provide clinical supervision to staff as required.
  • To be accountable for undertaking mandatory/essential training as per MCHFT guidelines.
  • To take responsibility for own continuing professional development and performance and maintaining own portfolio in accordance with the re-registration requirements
  • To identify own development needs in relation to current practice and future plans and setting personal development objectives
  • To attend relevant courses/meetings and mandatory in-house training as directed
  • To assist the Lead Trainer with professional training/updating programmes to assist service needs/developments.
  • To participate in the Trust’s performance appraisal system and to undertake any identified training and development related to the post.
  • Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.

RESEARCH/AUDIT

  • Participate in the evaluation of current practice and facilitate further discussion and review as appropriate.
  • Identify those elements of current clinical research that may be relevant to the service and assist in their implementation using a questioning and analytical approach to care.
  • Participate in the Trust’s research and audit projects as required, ensuring that any beneficial results are implemented on an agreed basis.
