You will be working as a part of our bespoke team, under the direction/supervision of nursing/medical staff and alongside family members, to provide agreed clinical interventions and planned, individualised care for children and young people with complex health needs within the home, school/day centre or hospital environment.

The service provides care 24 hours a day, 7 days a week; in a combination of evening shifts, night shifts and day shifts and covers weekends and Bank Holidays creating an environment which allows our team to maximise their ability and ensuring that the care delivered is appropriate for the needs of the children, young people and the families we support.

Main duties of the job

  • An individual with the ability to work flexibly within an internal rotational shift pattern including early, late, long-day and night shifts as well as weekends and Bank Holidays.
  • Those who wish to join a community team that provides bespoke packages of care; the Children’s and Young People’s Complex Care Package (CYPCCP) service provides care and support for children and young people with complex health needs from birth to 19 years of age or 25 years of age where a specific need is identified.
  • Those with an interest in acquiring clinical skills and knowledge specific to our area, working with children and young people who have profound multiple learning disabilities and complex health needs, and supporting them through important transitions to ensure they have the opportunity to fulfil their potential.
  • An individual who will work closely with our Team Leaders, the child and family, and multi-professional colleagues to develop relevant skills in order to deliver the necessary specialised care required.
