This is an exciting and unique opportunity to work as an advanced diabetes dietitian providing high quality care to young adults with diabetes. You would be working alongside a highly specialist, patient-centred diabetes service and would be part of a wider Nutrition and Dietetics department who are a friendly, supportive and forward-thinking team.
Main duties of the job
- As an HCPC registered Dietitian, provide expert dietetic service to the MDT and a caseload of primarily transition-aged diabetes patients.
- Engage with, advise and train other healthcare professionals involved in the care of patients with diabetes.
- Work as a senior member of the Dietetic Department, in the promotion of a high-quality patient-focused service and contributing to service development and cover.
Detailed job description and main responsibilities
- Autonomously provide expert, holistic, patient-centred dietetic care to individuals with diabetes, particularly the transition age diabetes population, underpinned by clinical, professional and Department guidelines and integrating with other services.
- Provide following specialist diabetes skills:
- Practical knowledge and skills required for insulin pump therapy for people with Type 1 diabetes
- Education to people with diabetes to enable them to adjust their insulin dosages and carbohydrate in order to achieve their targets for blood glucose control.
- Education regarding technology and data interpretation for example continuous blood glucose monitoring.
- Identification of appropriate treatment aims for people with type 2 diabetes using blood glucose-lowering agents and explain their mechanisms of action. Working with the multidisciplinary team to make suggestions to the suitable glucose-lowering agents.
- Provide the transition service and if required, contribute to the cover of:
- Secondary care clinics and outreach
- BERTIE and Living With Diabetes (for newly diagnosed type 1 diabetes) group education.
- Antenatal clinics
- Inpatient care at Dorset County Hospital.
- Indirect patient care via support to secondary care staff e.g. MDT meetings, Advice & Guidance enquiries, support to other DCH dietitians.
- Be an integral part of the wider diabetes and dietetic teams within DCHFT and where competent to do so, support other services offered by the teams as and when the need arises.
- Aid service resilience and skill maintenance by exchanging cover with the other diabetes dietitians as needed.
- Actively manage the risks and benefits of own interventions within the MDT context.
- Monitor and evaluate the clinical effectiveness of the specialist diabetes dietetics service to ensure quality and justify change.
- Maintain and progress own knowledge and skills to ensure high quality and holistic dietetic care to patients with diabetes, who may have a number of comorbidities.
- Ensure patient involvement in the development of the service.
- Participate in or lead MDT meetings and case conferences as required.
