Advanced Clinical Practitioner required for Stamford Gp Out of Hours Service to assess, treat, refer and discharge minor illness for patients with a booked appointment. An understanding and management of long-term conditions would be an advantage for this post.
Competence in Telephone triage.
Prescribing, and reviewing the use of safe effective medication appropriately; in line with best guidance and evidence-based practice.
Request, review and interpret appropriate investigations.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and partnership or organisation.
There will be opportunities to support home visiting as required and urgent treatment centre in LCHS
This is a Bank position working with our friendly team of Gps and Health Care Support workers.
Stamford Gp out of hours opening times.
Monday - Friday. 19.00-22.00
Saturday, Sunday and Bank Holiday. 08.00- 22.00
The Advanced Clinical Practitioner role works beyond normal professional boundaries as a Registered Nurse and will be working at an advanced level within the four ACP pillars of clinical practice, leadership and management, education and research. You will be expected to be autonomous in assessing and managing patients attending the Gp Out of Hours service.
