As a Registered Nurse your work will be split between the RightCare call centre based at the Kendray Hospital site and Barnsley Hospital. Whilst in the call centre you provide Clinical advice to referrers, assess and triage patients, making onwards referrals to the correct level of care.

You will be responsible for contacting Virtual Ward patients via telephone, digital monitoring, assessing and participating in the virtual MDT meetings where patients are discussed with Community, Hospital and Social Care colleagues.

Your work will be rostered and split between RightCare at the Kendray Hospital site and Barnsley Hospital where you will be assessing patients who have been referred to the Virtual ward and processing their admission. As part of the national Operational Planning Guidance the introduction of Virtual Wards (VW) across system is seen as a priority for the NHS. Initially, the focus is for each PLACE to develop a VW to support patients with Acute Respiratory Infection (including Covid patients) and Frailty in their routine place of residence. The overall national ambition is for systems to provide between 40-50 virtual ward beds per 100k population by December 2023 for Barnsley this equates to 106-132 beds. The expectation is that VWs will be operational seven days a week, 12 hours a day, with an out of hours provision for any patients on the VWs that require support. This is a very exciting opportunity to be involved in its ongoing development.

Main duties of the job

You will assist the Operational Manager in the development of RightCare Barnsley and Virtual Wards, to ensure safe, effective and individualised support to patients discharged early onto a Virtual Ward or stepped up to support hospital avoidance from a community setting.

You will consistently maintain quality patient care facilitating improvements and ensuring a good patient experience and manage daily operations of caseloads, successfully onboard patients onto the service, triage new referrals and support patient escalation when necessary.
