Southampton Neonatal Services are looking to recruit an passionate RN / RM with experience in neonatal nursing to join our Home Team as a band 6 Neonatal Outreach Sister.
Southampton Neonatal Services care for patients and their families across the South Coast region including the channel islands, providing intensive care, high care and special care, as well as having a Transitional Care Unit and a community outreach team to support discharge planning and care in the home. We host the regional transfer service, SONeT, ensuring that families receive care in the most appropriate hospital, closest to their home. We are a tertiary referral centre able to offer a range of specialities to our patients across the Thames Valley and Wessex region, including neonatal surgery and cardiac services.

Supporting to lead the established Neonatal Home Team, you will participate in assessing, planning, implementing, and evaluating individualised discharge plans for our neonatal and transitional care patients and their families. Working with the senior multidisciplinary team, you will anticipate discharges, co-ordinate arrangements and follow up plans. You will support families with the transition from hospital to home, providing education and emotional support as necessary. You will continue to provide holistic family-centred care to patients in the home environment in partnership with their families, whilst supporting junior staff to do the same.
What we’re looking for... * A registered nurse or midwife with neonatal experience who has completed the qualified in speciality course. * Experience of working in the community * Excellent organisational skills and the ability to use own initiative * Ability to work effectively as a lone worker and as part of a multi-professional team * Able to work under pressure with excellent time management and leadership skills * Full clean drivers licence (manual)
